Usage and nuance
As a plain pronoun 彼女 sounds a little formal; people often say あの人 or use the name instead. In narration, subtitles and serious dialogue it is the normal word for "she".
The girlfriend meaning is everywhere in romance anime and even in titles, like Kanojo, Okarishimasu (Rent-a-Girlfriend). 彼女いるの? is "Do you have a girlfriend?"
The male pair is 彼 (kare), "he" or "boyfriend", though for "boyfriend" people more often say 彼氏 (kareshi).
